Submit Your Immunization Forms On Med+Proctor

All incoming students are required to submit their immunization forms on Med+Proctor. Uploading your documents is easy! Follow the steps below.

Failure to complete all items will cause a delay in your ability to register for classes.

Med+Proctor Steps

Step 1: Register

Visit and click "register" to start a new account. There is a one-time $10 set up fee to utilize the service.

You must use your Howard email address ( and your student ID number must include the "@" symbol.

Step 2: Download

Fill out any required personal, medical, or insurance information. Download the Required Med+Proctor forms and follow the instructions provided.

Takes your forms to your medical professional so they can complete and sign the forms.

Do not bring, fax or email any medical forms to the Student Health Center.

Be Sure Your Health Profressional Completes These Items on Your Forms

ALL students, regardless of age, are required to be screened for Tuberculosis (TB). 

In addition, in accordance with D.C. Law 3-20, students under the age of 26 by AUGUST 1st (fall semester) and December 1st (spring semester) must provide documentation of vaccination or immunity from Hepatitis B, Diptheria, Tetanus, Pertussis, Measles, Mumps, Rubella and Varicella. YOU MUST USE THE MEDPROCTOR IMMUNIZATION CERTIFICATION AND PHYSICAL EXAMINATION FORM ALONG WITH ANY NECESSARY LAB OR RADIOLOGY REPORTS.

  • Tuberculosis screening.  If you do a PPD test, make sure the form lists the date(s) the test was placed, the date it was read and the result.  If you had a blood test or x-ray, make sure the official result is submitted with your immunization form.  We do not accept handwritten results. Please upload a copy of the lab or x-ray report.
  • Make sure your healthcare provider prints their name, signs, and dates the form and includes their NPI number.
  • Be sure the names of the vaccines match what is on our form, and that it is signed and stamped by that office.
  • Tetanus/Diphtheria vaccines are seemingly similar but there is a difference between Td, Dt, Tdap, and DPT vaccines.  Make sure you list the right ones!  We require Tdap.
  • If you do not have access to your immunization history, you can ask your doctor to draw immunity titers for MMR, Hepatitis B, and Varicella.  You still need to satisfy the requirements for Tuberculosis Screening, Tetanus/Diphtheria & acellular Pertussis.
  • Meningococcal vaccination is required for all first time in college and/or will live in residence halls.  Booster should be on or after 16th birthday.
  • Physical examination within the past year
  • Your school or college must match with the University’s records.  Please review and select the correct school/college.

Step 3: Upload

Log into your Med+Proctor account.

Upload the required Med+Proctor immunization form, required Med+Proctor Physical Examination Form, and laboratory/radiology reports as applicable. You can even take pictures of the forms with your smartphone to upload them.

Be sure all forms are filled out in their entirety, including dates and signatures.

Make sure the Health Professional areas are completed and dates are legible.

You will receive an email confirmation once Med+Proctor receives the forms.

After You Upload

Once Med+Proctor verifies that your forms are complete, they will inform the Howard University Student Health Center. When the Student Health Center receives that notification, they will take the steps to remove the medical hold from your student account.

Med+Proctor will also send you an email update stating that your forms are "verified as complete."
