Requirements For Students
It is incumbent upon each student to provide all required documentation in a timely manner as part of their pre-enrollment package. The Student Health Center is committed to creating a seamless transition through meeting your medical requirements, so please review the information on this page very carefully. We have partnered with Med+Proctor for verification of your immunizations and medical requirements for Howard University. This system is a cloud-based, fully automated and user-friendly platform that is both HIPAA and FERPA compliant.
Once you have paid the enrollment fee and you have been assigned an HU email and student ID number, please log onto Med+Proctor and set-up an account. There is a one-time $10 set-up fee to utilize the
service. Your information will be secure and comes with a lifetime membership. We are confident this platform will streamline your clearance and allow you to focus on preparing for entry into Howard University.
All new, transfer, and former returning students attending the University must utilize Med+Proctor for collection and verification of health information and student immunization. Log on to Med+Proctor using the instructions in the diagram to the right in order to complete the questions, which will help to establish the requirements specific to your school or college. You can download the medical forms and take them to your medical provider. Be sure all forms are filled-out in their entirety, including dates and signatures. Next, you will upload your forms to the Med+Proctor site. Do not bring, fax, or email any medical forms to the Student Health Center. All of your documents must be uploaded to Med+Proctor. With this format, you can even upload pictures from your smart phone or mobile device.
Please note, the deadlines for registration are July 1, for the fall semester and December 1, for the spring semester. You must use your Howard University Student ID number and Bison email to submit documents. Please contact Med+Proctor via messaging or using the live chat feature (bottom right of the screen) if you have any questions/concerns.
